Working together with the you, your family and your physician, the nurses at VNLV develop a plan of care to meet your individual needs. By providing clinical care, patient and family education and support, our nurses coordinate a home care program to help you become independent in managing your care safely at home.

Cardiopulmonary
Care
VNLV offers a comprehensive cardiopulmonary program tailored to meet the needs of patients with a wide range of cardiac and pulmonary conditions. Whether recovering from an acute cardiac event or managing chronic heart disease or COPD, our interdisciplinary team, comprised of RNs, PTs, OTs, and MSWs, has the expertise to develop individualized care plans that promote optimal recovery and quality of life.

Diabetes
Management
Diabetes is a chronic, incurable condition that requires daily management through your doctor’s guidance, a healthy diet, regular exercise, and ongoing education. Proper management helps reduce the risk of complications, including heart and circulatory disease.
At VNLV, our nurses are highly trained and knowledgeable in diabetes care. They work closely with patients and families to provide education, support, and practical strategies for managing the condition, helping patients maintain better health and improve their overall quality of life.

Integrated Chronic Care Management
VNLV’s Chronic Care Management Program empowers patients and families to understand and manage their conditions, helping to maximize quality of life, reduce unnecessary hospitalizations, and control pain and other symptoms. Our approach emphasizes holistic care that addresses the body, mind, and spirit, and we create individualized care plans tailored to each person’s unique needs and personal goals.

Rehabilitation
Therapy
Physical Therapy (PT)
Physical therapy focuses on improving mobility, strength, balance, and overall function. Our licensed physical therapists work with patients to restore movement, reduce pain, prevent injury, and help them safely perform daily activities. Treatment may include individualized exercise programs, manual therapy, gait training, and therapeutic modalities tailored to each patient’s needs and prescribed by a physician.

Occupational Therapy (OT)
Occupational therapy helps patients regain independence in daily activities affected by injury, illness, or chronic conditions. Our occupational therapists teach adaptive strategies, provide specialized exercises, and recommend equipment or modifications to support activities such as dressing, cooking, work tasks, and self-care, helping patients live as fully and safely as possible.

Speech Therapy (ST)
Speech therapy addresses communication, swallowing, and cognitive-linguistic challenges. Our speech-language pathologists evaluate and treat difficulties related to speech, language, cognition, voice, and swallowing (dysphagia). Treatment may include exercises to improve articulation, language comprehension and expression, cognitive skills, and safe swallowing techniques, supporting patients’ independence and quality of life.

Specialized Wound and Ostomy Care
Wound care is an increasingly important need for home care patients. At VNLV, our Wound Care Nurses utilize advanced technology and innovative wound care products to achieve optimal outcomes. Overseen by a certified Wound and Ostomy Nurse, our program is designed to promote healing and comfort while providing patients with reassurance and peace of mind.

Medical Social Worker and Home Health Aides
Medical Social Work (MSW)
VNLV’s medical social workers provide compassionate support to help patients and families navigate the healthcare system and connect with community resources. They conduct thorough assessments of each patient’s emotional, social, and practical needs, then develop individualized care plans. Services include supportive counseling, education, and referrals to help patients cope with illness, injury, or life transitions, promoting overall well-being and quality of life.

Home Health Aides (HHA)
Our trained and credentialed home health aides assist patients with daily activities to maintain independence and comfort at home. Services include help with bathing, dressing, grooming, mobility, and other personal care tasks, delivered with dignity, respect, and a focus on safety and comfort.
Payment Options
We accept Medicare and most major insurance plans. While we strive to accommodate a wide range of coverage, not all plans are accepted. Please contact us to confirm whether your insurance is covered.
Medicare: Covers 100% of home health services for individuals 65 and older when the following criteria are met:

Services are medically necessary and ordered by a physician.

The patient is homebound, meaning leaving home requires considerable effort and occurs only infrequently and for short periods.

At least one skilled service is required (nursing or physical therapy).

Services are provided on a part-time or intermittent basis (not 24/7 care).
Medicaid: This state program provides home health services for individuals with limited income who have no other means of coverage. Contact the Connecticut Department of Social Services for eligibility information.
Private Insurance: VNLV accepts most major insurance plans. Please contact us directly to verify coverage.
Self-Pay: For services not covered by insurance or other funding programs, clients may pay directly. Contact VNLV to learn more about payment options.
