Role Overview:
We are excited to launch our Outpatient Physical Therapy in the Home program, a new and innovative service line under Medicare Part B. We’re looking for forward-thinking, motivated Physical Therapists who want to help shape this pilot program from the ground up. This position is ideal for clinicians who value autonomy, creativity, and a hands-on role in improving patient access to outpatient-level therapy. This position will be per-diem/part-time to start, with the potential to move into full-time as our caseload grows.

Responsibilities:

  • Provide outpatient-level therapy in patients’ homes.

  • Evaluate, plan, and implement individualized treatment programs.

  • Collaborate with patients, families, and interdisciplinary team members to ensure quality care.

  • Help develop and refine workflows and best practices as the program grows.

Qualifications:

  • Current Connecticut Physical Therapy license (or eligibility to apply).

  • Graduate of an accredited PT program.

  • Strong communication and organizational skills.

  • Self-starter with a collaborative, solution-oriented mindset.

  • Basic technology literacy (EMR use, tablets, telehealth, documentation).
