Outpatient Physical Therapist
in the Home

Role Overview:
We are excited to launch our Outpatient Physical Therapy in the Home program, a new and innovative service line under Medicare Part B. We’re looking for forward-thinking, motivated Physical Therapists who want to help shape this pilot program from the ground up. This position is ideal for clinicians who value autonomy, creativity, and a hands-on role in improving patient access to outpatient-level therapy. This position will be per-diem/part-time to start, with the potential to move into full-time as our caseload grows.
Responsibilities:

Provide outpatient-level therapy in patients’ homes.

Evaluate, plan, and implement individualized treatment programs.

Collaborate with patients, families, and interdisciplinary team members to ensure quality care.

Help develop and refine workflows and best practices as the program grows.
Qualifications:

Current Connecticut Physical Therapy license (or eligibility to apply).

Graduate of an accredited PT program.

Strong communication and organizational skills.

Self-starter with a collaborative, solution-oriented mindset.

Basic technology literacy (EMR use, tablets, telehealth, documentation).
