Per Diem Registered Nurse

Role Overview:
Visiting Nurses of the Lower Valley (VNLV), a non-profit home health agency located in Centerbrook, CT, is now hiring a Per Diem Registered Nurse to work weekdays. Duties include, but are not limited to: client assessment, OASIS timepoints, Home Health Aide supervision, and functioning as part of a collaborative, patient-centered care team.

You’ll provide skilled nursing care to patients in their homes and work closely with physicians, therapists, and other healthcare professionals to support patient recovery and independence.

Responsibilities:

Perform comprehensive patient assessments, including OASIS documentation at start of care, recertification, resumption of care, and discharge.

Develop and implement individualized care plans in collaboration with the healthcare team.

Provide direct nursing care, including wound care, medication management, and disease education.

Supervise and coordinate care with Home Health Aides and other support staff.
-
Communicate effectively with patients, families, and the care team to ensure continuity of care.

Maintain accurate clinical documentation in accordance with agency and regulatory standards.

Participate in case conferences and ongoing training to support quality care delivery.

Qualifications:

Graduate of an accredited school of nursing.

Current RN license in the State of Connecticut.

Minimum of one year of recent med-surg or acute care clinical experience.

Home health care and OASIS experience strongly preferred (but not required).

Reliable transportation and valid driver's license.

Strong organizational and communication skills.

Ability to work independently and manage time effectively.

Compensation:

Pay per visit: $50 - $125, depending on visit type and complexity.
