Role Overview:

Visiting Nurses of the Lower Valley (VNLV), a non-profit home health agency located in Centerbrook, CT, is now hiring a Per Diem Registered Nurse to work weekdays. Duties include, but are not limited to: client assessment, OASIS timepoints, Home Health Aide supervision, and functioning as part of a collaborative, patient-centered care team.

You’ll provide skilled nursing care to patients in their homes and work closely with physicians, therapists, and other healthcare professionals to support patient recovery and independence.

​

Responsibilities:

  • Perform comprehensive patient assessments, including OASIS documentation at start of care, recertification, resumption of care, and discharge.

  • Develop and implement individualized care plans in collaboration with the healthcare team.

  • Provide direct nursing care, including wound care, medication management, and disease education.

  • Supervise and coordinate care with Home Health Aides and other support staff.

  • Communicate effectively with patients, families, and the care team to ensure continuity of care.

  • Maintain accurate clinical documentation in accordance with agency and regulatory standards.

  • Participate in case conferences and ongoing training to support quality care delivery.

Qualifications:

  • Graduate of an accredited school of nursing.

  • Current RN license in the State of Connecticut.

  • Minimum of one year of recent med-surg or acute care clinical experience.

  • Home health care and OASIS experience strongly preferred (but not required).

  • Reliable transportation and valid driver's license.

  • Strong organizational and communication skills.

  • Ability to work independently and manage time effectively.

Compensation:

  • Pay per visit: $50 - $125, depending on visit type and complexity.
