Per-Diem/Part-Time/Full Time Homecare Physical Therapist

Role Overview:
We are seeking a dedicated and compassionate Homecare Physical Therapist to join our skilled clinical team. This role offers the opportunity to make a meaningful difference by helping patients recover safely and independently in their own homes under Medicare Part A. Ideal candidates are motivated clinicians who value autonomy, patient-centered care, and the flexibility that homecare provides. This position will be per-diem/part-time to start, with the potential to move into full-time as our caseload grows.

Responsibilities:

• Evaluate, plan, and implement individualized treatment programs for patients in their homes.

• Collaborate with nurses, occupational therapists, speech therapists, and other team members to ensure coordinated, high-quality care.

• Educate patients and caregivers on exercise programs, safety, and mobility strategies.

• Document all care accurately and promptly within the EMR system.

• Participate in case conferences and communicate effectively with referring physicians and agency staff.

Qualifications:

• Current Connecticut Physical Therapy license (or eligibility to apply).

• Graduate of an accredited Physical Therapy program.

• Strong clinical reasoning, communication, and time-management skills.

• Compassionate, independent, and detail-oriented.

• Basic technology literacy (EMR, tablets, telehealth, documentation).
