Nursing - Adjectives such as knowledgeable, kind, dedicated, caring, and patient, are often used to describe our nurses. With an average of 19 years of home health experience, our nurses work diligently with the patient to control the impact of acute and chronic illnesses, such as diabetes and heart disease, and to reduce complications caused by wounds and ostomies. Equally important, they teach self-care techniques to patients and their caregivers, which foster independence and enhance healing.  [BACK TO TOP]

Diabetes Care - Managing diabetes can help reduce the risk of long-term complications such as heart disease, stroke, kidney or nerve damage and vision problems. VNLV nursing can help with steps to control the disease by adjusting lifestyle and diet, monitoring blood sugar, and adhering to medication regimens.  [BACK TO TOP]

Cardiac - Heart disease affects 81 million people and is the leading cause of death for both men and women in the United States. VNLV nursing has a specialized Cardiac Step Program, which works with the treatment plan recommended by the doctor to improve health and prevent complications. VNLV cardiac care can include monitoring blood pressure and heart rate for medication adjustments, teaching self-management techniques, assessing the efficacy and side effects of all medications and providing rehabilitation services.  [BACK TO TOP]

Psychiatric - Medication management and supportive counseling are provided to those individuals with persistent mental illness. Using a clinical case management approach, our psychiatric nurses monitor client progress and maintain communication with physicians and community mental health clinicians.  [BACK TO TOP]

Wound Care - At VNLV, we aim to provide the highest level of wound care for our patients. A skilled nurse will work closely with the patient, family and care team to establish a treatment plan and evaluate progress every step of the way. While specific treatments vary by patient, our nurses can provide physician-ordered wound care, self-care training, and work closely with area-wound care facilities.  [BACK TO TOP]

Medication Management - VNLV nursing can help make sure patients are taking the right dose on the right day at the right time.  VNLV staff pre-pour or pre-measure medications, check to make sure prescriptions are filled, expiration dates are noted, and monitor for potential side effects. They also work with doctors and pharmacists to prevent dangerous drug interactions, assess the effectiveness of medications, and develop reminder systems and extra safety precautions.  [BACK TO TOP]

Maternal and Child Health - For 90 years VNLV has provided compassionate and expert care to mothers and children. We provide care to pregnant women with prenatal or medically-related complications, new mothers, and babies and children up to 18 years of age. Services include pediatric nursing, therapy and social work, as well as breastfeeding consultation and daycare center consulting.  The emphasis is on teaching family members to take care of the child's needs.  [BACK TO TOP]

Social Work -  VNLV social workers team with nurses, physical therapists, occupational therapists, speech therapists, nutritionists, and home health aides to assist patients and families to cope with illness.  A social worker’s role may include advocating for patients, referring them to supportive community services and providing benefits counseling.  Our social workers are expert at guiding the patient and family through our complex medical system.  [BACK TO TOP]

Home Health Aides - Our staff of credentialed, experienced and well-trained home health aides provide critically needed support services, including assistance with bathing, dressing, meal preparation, feeding, light housekeeping and laundry. A registered nurse supervises the home health aide, and services are closely monitored for appropriateness and quality. Occupational Therapy - A stroke, illness, or injury can impact fine motor skills and ability to care for one's self.  Occupational therapy can help make sure patients regain the ability to perform everyday activities such as eating, dressing, and bathing, as well as preparing a meal or enjoying a favorite leisure activity. VNLV occupational therapists will help patients improve fine motor skills, adapt their day-to-day routines, learn strategies or the use of specially designed equipment to complete tasks and activities safely, and speed recovery through exercises and positioning programs.  [BACK TO TOP]

Physical Therapy - VNLV physical therapy services are often needed for patients who have suffered a stroke, had a joint replacement, a chronic illness or a neurological condition such as Parkinson’s disease. Physical therapists can help patients improve their strength, flexibility, balance and coordination. VNLV in-home therapists design a program specific to patient needs, which includes strategies to help move from bed to chair; walk on level ground or navigate stairs; use a cane or walker safely and effectively; improve posture and balance; and learn to prevent falls.  [BACK TO TOP]

Speech Therapy - Speech therapy services may be needed to assist patients to relearn or improve their ability to form words. Additionally, if a patient has suffered a stroke or other illness that affects the muscles that control swallowing, a VNLV speech and language pathologist can help develop an individualized program to ensure speaking and swallowing are easier and safer.  [BACK TO TOP]
